Troop VA9020 Documents and Forms

• 2021-2022 Policies & Guidelines Handbook - Our Troop's current Policies & Guidelines Handbook.
• Service Log - Use this form to track all service completed during the AHG program year (and the summer prior to the start of the program year). This form should be used to track service time earned with the Troop and also outside of Troop time.
• Badge/Sports Pin Report - Use this form to track all badge work done at home - either for make-up work for an in-unit badge, or for at-home badges. This form is also used to indicate completion of sports pin requirements.
• Visitor Troop Meeting Permission Slip - Collect this form from families visiting Troop Meetings.
• Special Event/Service Project Attendance Log - Use this form to track attendance at Troop special events/service projects.
• Health and Medical Form - We keep one on file for every registered girl and adult member.
• Request for Administration of Medication Form - We keep one on file for every registered girl member. 
• High Adventure Activity Form - Registered girl and adult members who will be participating in Authorized High Adventure Activities are required to complete a new High Adventure Activity Medical Form each year. These are to be filled out by a lisenced health care provider.
